Here are the two forms that we need to have before members can ski and snowboard with us.
For minors, the forms must be filled out by the minor's legal guardian. The Medical Release form also can have a picture attached to it. The recent picture helps in knowing what our participants look like for safety purposes.

You can also present these documents in person either when in contact with program personel or at the time of the first ski trip you take with us.A member cannot ski with us if they do not have these forms properly filled out.
